Survey - Governor's Council on Developmental Disabilities

The Governor's Council on Developmental Disabilities must collect some
information from individuals who have used Council services or products,
or who have participated in any Council funded projects, activities,
initiatives or opportunities, ("consumers" of our activities). Examples
of these would be Partners in Policy-making, Disability Day at the
Capitol, Partnership Funds, Voices that Count, etc.

The Council has prepared an On-Line survey that is very short and easy
to complete. Please take a few minutes to complete the survey at
http://www.surveymonkey.com/s.asp?u=510792571882

We are collecting this information to meet the requirements of our
federal grant from the Administration on Developmental Disabilities, and
we must report this information to them in January. Your responses are
anonymous. There is no way for us to determine personal identification
for those who respond. There is also room for you to record feedback
that may not be included in the questions.
